The Point's Labor Request Form
Complete this form in its entirety for such things as:
set up, construction or pick-up. (At least 2 weeks in advance). NOTE: IF YOUR MINISTRY BUILDS IT, YOUR MINISTRY NEEDS TO DISASSEMBLE AND STORE IT IN ITS PROPER PLACE.
If purchased item(s) need to be picked up, you will be required to attach a receipt to the confirmation sent to you.
Also, if an item needs to be constructed, attach the diagram to the confirmation sent to you.
Set up: While Facilities will assist in setting up for your event, you will be required to restore items to their normal setting, including vacuuming, emptying trash, wiping down common areas used, such as tables, counters, etc.
Construction: Give the dimensions, type of material, colors & attach a diagram of your desired construction. Please list the Budget expense code as well.
Pick-up: Please give the address, time of pick-up, person at the location & where you would like the item(s) placed on CPCC property.
Date Needed (Minimal of 2 weeks) The Operation staff has routine tasks & must have ample time to work additional requests into their schedule.
